There is limited consensus about what constitutes humanly sensitive care, or how it can be sustained in care settings. A new humanised care assessment tool may point to caring practices that are up to the task of meeting persons as humans within busy healthcare environments. This paper describes qualitative development of a tool that is conceptually sensitive to human dimensions of care informed by a life‐world philosophical orientation. Items were generated to reflect eight theoretical dimensions that constitute what makes care (...) feel humanly focused. An action research group process in 2014–2015 with researchers, service users, healthcare professionals in two diverse clinical settings (stroke rehabilitation and dermatology) was used. Feedback on conceptual content, transparency of meaning and readability was then gained from a panel in Sweden and third‐year student nurses in the UK. The tool can be applied to attune staff to human dimensions of care, offering items which point to concrete examples of humanising and dehumanising features of practice in ways that have not yet been fully captured in the caring literature. Based on theoretically led experiential items, with dedicated focus on what makes people feel more, or less than human, it may offer improvement on available assessments of care. (shrink)

Understanding people's experience of skin ageing as it is lived can enable sensitive approaches to promoting healthy skin and to care in general. By understanding the insider perspective, what it is like for individuals, a way to sensitise practice for more humanly sensitive care is offered. Through interviews with seventeen community‐dwelling older people, the essential meaning of living within ageing skin was illuminated as a state of managed inevitability. The skin is inevitably changing, and ageing skin is a marker of (...) change over time but the person within remains. Constituents of the phenomenon comprise the experience of unfamiliar sights and sensations given by ageing skin; facing and accepting bodily changes and seeing this back and forth in family connections; taking care of the skin “to face” the world; and to present oneself to others and a different place in the world, same person, changed body. Findings point to why and how nurses can treat older people as persons by not over emphasising a view on ageing bodies or bodies with aged skin alone, but in tempering this view with deeper existential insights, meeting the older person with a skin care need as a person and not just as a physical entity. ABSTRACT The aim of this article is to bring philosophical thinking closer to practice-based empirical work. Using Martin Heidegger’s philosophy, it offers a bridge between these two worlds, attempting to provide philosophical depth to the findings of a hermeneutic phenomenological study. This process unfolded through the appearance of three intertwined, potential, meaningful modes of being in the lifeworld: space as a condition for being and being for worlding the world; temporal and spatial self-being, the existence of multiple selves in time (...) and space; and suffering and thriving as modes of being. Background Health ministries in Europe are investing increasingly in innovative digital technologies. Older adults, who have not grown up with digital innovation, are expected to keep up with technological shifts as much as other age groups. This is ethically challenging, as it may threaten a sense of dignity and well-being in older adults. Research objective To clarify the phenomenon of sense of dignity experienced in older adults, concerning how their expectations and needs are met within the context of digitally led (...) healthcare in Norway. Research design A Reflective Lifeworld Research design was chosen, and purposive, in-depth interviews were conducted. Participants and research context The participants were 13 adults 75 years and older from Northern Norway, living at home and not receiving consistent assistance. Ethical considerations Followed the principles of the Helsinki Declaration. This study was approved by the Social Science Data Services in Norway (project number 916119). Interviews were conducted carefully within a safe environment chosen by the participants. Findings Older adults experience that using new digital systems in healthcare makes them become dependent with experiences of helplessness. They feel an increased sense of dependency on other people, and that recognition can assail their experience of personal dignity. Older adults not only expect digitally led healthcare to give them a feeling of safety but also experience feeling insecure concerning privacy and loss of possibilities for dialogue with healthcare providers. They are met by demands from society, which they often struggle to achieve. Conclusion The phenomenon of sense of dignity experienced in older adults, concerning how their expectations and needs are met within digitally led healthcare, indicates a sense of feeling lost in the digital world. Further, innovative healthcare lacks focus on ethical performance. This impacts their perception of dignity, as loss of dignity is noticed especially in its rupture. (shrink)

Home care is one of the fastest‐growing industries in the United States, providing valuable opportunities for millions of older adults and people with disabilities to live at home rather than in institutional settings. Home care workers assist clients with essential activities of daily living, but their wages and working conditions generally fail to reflect the importance of their work. Drawing on the work of Eva Feder Kittay and other care ethicists, we argue that good care involves attending to the needs (...) of another out of a concern for their well‐being. Such care should be standard in the home care system. Yet, because of the pervasive racial, gender, and economic inequalities that the home care industry perpetuates, home care workers and their clients cannot reasonably be expected to care about each other. We endorse reforms aimed at enabling home care workers and their clients to form and maintain professional relationships that cultivate care.. (shrink)

This qualitative study describes the lived experience of physicians who work in communities that have experienced a public mass shooting. Semi-structured interviews were conducted with seventeen physicians involved in eight separate mass casualty shooting incidents in the United States. Four major themes emerged from constant comparative analysis: The psychological toll on physicians: “I wonder if I'm broken”; the importance of and need for mass casualty shooting preparedness: “[We need to] recognize this as a public health concern and train physicians to (...) manage it”; massive media attention: “The media onslaught was unbelievable”; and commitment to advocacy for a public health approach to firearm violence: “I want to do whatever I can to prevent some of these terrible events.”. Mobile devices with health apps, direct-to-consumer genetic testing, crowd-sourced information, and other data sources have enabled research by new classes of researchers. Independent researchers, citizen scientists, patient-directed researchers, self-experimenters, and others are not covered by federal research regulations because they are not recipients of federal financial assistance or conducting research in anticipation of a submission to the FDA for approval of a new drug or medical device. This article addresses the difficult policy challenge of promoting the welfare and interests of (...) research participants, as well as the public, in the absence of regulatory requirements and without discouraging independent, innovative scientific inquiry. The article recommends a series of measures, including education, consultation, transparency, self-governance, and regulation to strike the appropriate balance. (shrink)

Researchers have long suspected that grapheme-color synaesthesia is useful, but research on its utility has so far focused primarily on episodic memory and perceptual discrimination. Here we ask whether it can be harnessed during rule-based Category learning. Participants learned through trial and error to classify grapheme pairs that were organized into categories on the basis of their associated synaesthetic colors. The performance of synaesthetes was similar to non-synaesthetes viewing graphemes that were physically colored in the same way. Specifically, synaesthetes learned (...) to categorize stimuli effectively, they were able to transfer this learning to novel stimuli, and they falsely recognized grapheme-pair foils, all like non-synaesthetes viewing colored graphemes. These findings demonstrate that synaesthesia can be exploited when learning the kind of material taught in many classroom settings. (shrink)

As philosophers of mind we seem to hold in common no very clear view about the relevance that work in psychology or the neurosciences may or may not have to our own favourite questions—even if we call the subject ‘philosophical psychology’. For example, in the literature we find articles on pain some of which do, some of which don't, rely more or less heavily on, for example, the work of Melzack and Wall; the puzzle cases used so extensively in discussions (...) of personal identity are drawn sometimes from the pleasant exercise of scientific fantasy, at times from surprising reports of scientific fact; and there are those who deny, as well as those who affirm, the importance of the discovery of rapid-eye-movement sleep to the philosophical treatment of dreaming. A general account of the relation between scientific, and philosophical, psychology is long overdue and of the first importance. Here I shall limit myself to just one area where the two seem to connect, discussing one type of neuropsychological research and its relevance to questions in the philosophy of mind and the philosophy of psychology. It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

The Oregon Health Division's report on assisted suicide under the state's new legislation does not provide enough data to support its reassuring conclusions. Especially glaring is the lack of data on the palliative care offered to patients who requested assisted suicide, on their financial situation, and on their emotional state.

The paper discusses the utility of the notion of consciousness for the behavioural and brain sciences. It describes four distinctively different senses of 'conscious', and argues that to cope with the heterogeneous phenomena loosely indicated thereby, these sciences not only do not but should not discuss them in terms of 'consciousness'. It is thus suggested that 'the problem' allegedly posed to scientists by consciousness is unreal; one need neither adopt a realist stance with respect to it, nor include the term (...) and its cognates in the sciences' conceptual apparatus. The paper briefly examines Nagel's [1974] article, since this presents the strongest counter to the thesis proposed. (shrink)

Heterogeneity within MDD has hampered identification of biological markers (e.g., intermediate phenotypes, IPs) that might increase risk for the disorder or reflect closer links to the genes underlying the disease process. The newer characterizations of dimensions of MDD within Research Domain Criteria (RDoC) domains may align well with the goal of defining IPs. We compare a sample of 25 individuals with MDD compared to 29 age and education matched controls in multimodal assessment. The multimodal RDoC assessment included the primary IP (...) biomarker, positron emission tomography (PET) with a selective radiotracer for 5-HT1A ([11C]WAY-100635), as well as event-related functional MRI with a Go/No-go task targeting the Cognitive Control network, neuropsychological assessment of affective perception, negative memory bias and Cognitive Control domains. There was also an exploratory genetic analysis with the serotonin transporter (5-HTTLPR) and monamine oxidase A (MAO-A) genes. In regression analyses, lower 5-HT1A binding potential (BP) in the MDD group was related to diminished engagement of the Cognitive Control network, slowed resolution of interfering cognitive stimuli, one element of Cognitive Control. In contrast, higher/normative levels of 5-HT1A BP in MDD (only) was related to a substantial memory bias toward negative information, but intact resolution of interfering cognitive stimuli and greater engagement of Cognitive Control circuitry. The serotonin transporter risk allele was associated with lower 1a BP and the corresponding imaging and cognitive IPs in MDD. Lowered 5HT 1a BP was present in half of the MDD group relative to the control group. Lowered 5HT 1a BP may represent a subtype including decreased engagement of Cognitive Control network and impaired resolution of interfering cognitive stimuli. Future investigations might link lowered 1a BP to neurobiological pathways and markers, as well as probing subtype-specific treatment targets. (shrink)

Why Don't You Just Talk to Him? looks at the broad political contexts in which violence, specifically domestic violence, occurs. Kathleen Arnold argues that liberal and Enlightenment notions of the social contract, rationality and egalitarianism -- the ideas that constitute norms of good citizenship -- have an inextricable relationship to violence. According to this dynamic, targets of abuse are not rational, make bad choices, are unable to negotiate with their abusers, or otherwise violate norms of the social contract; they (...) are, thus, second-class citizens. In fact, as Arnold shows, drawing from Nietzsche and Foucault's theories of power and arguing against much of the standard policy literature on domestic violence, the very mechanisms that purportedly help targets of domestic abuse actually work to compound the problem by exacerbating the power differences between the abuser and the abused. The book argues that a key to understanding how to prevent domestic violence is seeing it as a political rather than a personal issue, with political consequences. It seeks to challenge Enlightenment ideas about intimacy that conceive of personal relationships as mutual, equal and contractual. Put another way, it challenges policy ideas that suggest that targets of abuse can simply choose to leave abusive relationships without other personal or economic consequences, or that there is a clear and consistent level of help once they make the choice to leave. Asking "Why Don't You Just Talk to Him?" is in reality a suggestion riven with contradictions and false choices. Arnold further explores these issues by looking at two key asylum cases that highlight contradictions within the government's treatment of foreigners and that of long-term residents. These cases expose problematic assumptions in the approach to domestic violence more generally. Exposing major injustices from the point of view of domestic violence targets, this book promises to generate further debate, if not consensus. (shrink)

Much recent attention has been paid to the Socratic elenchus, with considerable focus given to the structure of the elenchus and its desired benefits for both Socrates and his interlocutors. In this paper I focus on one of these benefits, namely the fostering of self-knowledge. I provide an examination of Socrates’ theory of self-knowledge and the way it is to be fostered through elenctic examination with an eye toward gaining afuller understanding of the foundations of our contemporary views. Though many (...) commentators routinely dismiss the Socratic view as overly intellectual, I maintain that “knowing what we know and don’t know” remains an important component of self-knowledge, even if it needs to be supplemented by an understanding of our more general psychological traits and capacities. Additionally, I argue that the Socratic focus on self-examination as a means to gaining self-knowledge remains a timely and important moral task. (shrink)

Some of the issues raised have been (1) Is the general point of view the moral point of view? (2) Is the general point of view necessary in order for a judgment to count as (...) a moral judgment? (3) Does the general point of view provide a justificatory perspective or is it a psychological explanatory concept that explains how moral judgments are made without necessarily offering a standard or normative basis for moral judgments? (4) If a general point of view yields some notion of impartiality, then how does it do so without introducing rationalistic elements that would undermine the sentimentalist approach Hume defends?1 In this paper I offer an interpretation of Hume's general point of view in morals as, employing a photographic analogy, a kind of focusing activity. It allows for strengthening of sentiments for those remote and, through contrariety, weakening of sentiments for those near such that we are moved to focus on those effects that are typical and, through conversing with others, arrive at general principles of praise or blame. I also examine the general point of view in comparison to what Hume has to say about regulation of belief. The comparison with belief sheds new light, I think, on how production of contrariety through the general point of view is regulative in morals. On my interpretation of the general point of view, it does not undermine Hume's sentimentalist thesis in morals, but rather is a device by which sentiment is properly aroused and directed. This will be helpful in setting up the scope of regulation in morals, since the latter involves, but is not limited to, regulation of belief. The discussion of belief will be followed by a longer discussion of the general point of view and the issue of regulation in morals. Finally, I will draw explicit comparisons between the two kinds of regulation that both illuminate moral regulation and secondarily suggest that Hume has a unified treatment of regulation. 1. The Generalizing Propensity and General Rules In Treatise 1.3.13 Hume explicitly discusses what he calls "general rules" (T.3.13.7-12; SBN 146-50). Four studies tested the hypothesis that a weaker belief in free will would be related to feeling less gratitude. In Studies 1a and 1b, a trait measure of free will belief was positively correlated with a measure of dispositional gratitude. In Study 2, participants whose free will belief was weakened (vs. unchanged or bolstered) reported feeling less grateful for events in their past. Study 3 used a laboratory induction of gratitude. Participants with an experimentally reduced (vs. increased) belief in free (...) will reported feeling less grateful for the favor. In Study 4, a reduced (vs. increased) belief in free will led to less gratitude in a hypothetical favor scenario. This effect was serially mediated by perceiving the benefactor as having less free will and therefore as being less sincerely motivated. These findings suggest that belief in free will is an important part of being able to feel gratitude. (shrink)

Testosterone fluctuates in response to competitive social interactions, with the direction of change typically depending on factors such as contest outcome. Watching a competition may be sufficient to activate T among fans and others who are invested in the outcome. This study explores the change in T associated with vicarious experiences of competition among combat sport athletes viewing a teammate win or lose and assesses how individual differences in social identification with one’s team relates to these patterns of T reactivity. (...) Twenty-six male combat athletes completed a social identity questionnaire on a neutral day. Later, salivary samples were obtained before and after athletes viewed a video of a teammate engaged in a formal contest. T reactivity to viewing a teammate compete varied among participants in both the magnitude and direction of change, independent of contest outcome. Individual differences in cognitive centrality, a core feature of social identification, showed a strong positive relationship with T reactivity, particularly if their teammate won. Initial findings suggest that dominance-linked androgen responses associated with watching a teammate win a competition might depend on the belief that team membership is central to one’s own identity. These exploratory results in a small sample of combat athletes should be interpreted with caution. Uncovering the role of social group dynamics in influencing T responses to competition is particularly important in light of the evolutionary history of coalitional combat in humans. (shrink)

In this paper I examine the frequently made claim that grasping a metaphor is a kind of ‘seeing-as’. I describe several ways in which it might be thought that metaphor-grasping is importantly similar to seeing-as, such that an extension of the latter category is though justified to include the former. For some of these similarities, I suggest they are illusory; for others, I argue that they are shared in virtue of the membership of both seeing-as and metaphor-grasping in some much (...) broader category, and so don’t obviously motivate thinking of metaphor-grasping as seeing-as. My aim is modest: not to deny that metaphor-grasping is a kind of seeing-as, but only to suggest that it should not be too quickly accepted. (shrink)

I have argued that there is a philosophical problem posed by a need to determine the reference of expressions which seem to refer to kinds of stuff or matter and to make identity claims about it (e.g., ‘the gold’, ‘the same clay’). Ordinary sortal expressions such as ‘lump’, and ‘piece’ have been shown to be inadequate to the task of providing reference for the expressions in question. What is necessary is an expression which does not have an ordinary sortal use (...) and which meets certain other conditions which I have enumerated. There does not seem to be an expression in ordinary language which fits these conditions. The technical term, ‘quantity’, as defined by Helen Cartwright, meet the conditions. ‘Parcel’, suggested V. C. Chappell, fails to meet the conditions. Another technical term, ‘m-elements’, coined by Laycock, comes close to meeting the conditions, but has a number of difficulties. Mose of these difficulties stem from the fact that counting (rather than measuring, as with quantities) is behind the application of ‘m-elements’, and stuff is as a matter of fact measured, rather than counted, in most situations. I have also considered a possible difficulty with ‘quantity’ (that in most cases in which someone talks of ‘same stuff’ strictly speaking we don't have the same quantity of stuff) and shown it not to be a genuine difficulty. In this paper I examine the moral import of apologizing within the U.S. legal system and as a response to historical acts of injustice. In both of these contexts apologies are expressed in a public forum, which adds an interesting dynamic to their moral significance. Within the legal system the (...) judge, representing the interests of the community, may use apologizing to directly address the harm done to victims, as an indicator of recidivism on the part of offenders, and to help create an atmosphere of respect for law in the community at large. Different moral aspects of apologizing come to the fore in the context of historical acts of injustice. Interesting philosophical questions arise, e.g. the legitimacy of moral judgments across time and culture and the very possibility of group action. And skepticism is commonly expressed about the value of such apologies: aren't they empty words that provide no real benefit to victims or the descendants of victims? Aren't they irrelevant to the future? I identify what I hope are convincing reasons to believe that historical apologies can in fact have considerable moral value and a significant impact on the future. (shrink)

Much as the quip “Isn’t business ethics a contradiction in terms?” ubiquitously accosts philosophers involved in that subdiscipline, “What’s a nice girl like you doing studying a misogynist like that?” has haunted my career in Nietzsche scholarship. I have never been entirely certain as (...) to whether this ad hominem is more directed at me or at Nietzsche.A sympathetic but jovial colleague played on this ambiguity by remarking, when he heard my topic for this essay, “Yes, that must be an interesting topic for someone who isn’t even shallow.” Yes, Nietzsche did say that women, far from deep, were not even shallow. But does this remark prove him a sexist? Are we even certain what it means?I will attempt to complicate our appraisal of Nietzsche’s alleged sexism by focusing on Book Two of The Gay Science. Considering his passages on women in that book, I will suggest along the way reasons why at least some readings of these as sexist should be rejected. More positively, I will show that these passages urge a reassessment of the relationship between men and women that can be seen as a contribution toward feminist theory (if an unwitting one), and that one of Nietzsche’s intended impacts on his male readers is to initiate a radical revolution in their thinking about women.IThe term “gender” is used advisedly in this context, for Nietzsche made the distinction, common in feminist discussion, between sex (the biological potential to play one role rather than another in [End Page 227] reproduction) and gender (the contingently assigned roles that a society attaches to those who are biologically male or female). Nietzsche urged his readers to recognize the contingency of gender roles and to consider the desirability of changing them.We can hardly begin to consider Nietzsche’s suggestions in these regards, however, without confronting the allegation that Nietzsche was a paradigmatic sexist. Although many of the most important names in Western philosophy have been singled out for feminist censure, Nietzsche is often held up as an exemplar of all that is misogynistic, both in the philosophical tradition and within patriarchy generally. Nietzsche is identified as a misogynist for a variety of reasons. He is seen as an essentialist, an opponent of women’s rights, an enthusiast of masculine virtue, and an advocate of male domination. While none of these attributions may be entirely apt, I agree with Nietzsche’s feminist critics that he was, at least sometimes, too willing to rest content with truisms about women and to vent personal rage in the guise of philosophy.That said, some of the attacks on Nietzsche are both unfair and unfortunate. I am not the only feminist to think so. Maudemarie Clark, Sarah Kofman, Maryanne Bertram, and Deborah Bergoffen have all suggested that certain of Nietzsche’s views offer starting points for feminist theorizing. I will describe some attacks made on Nietzsche by certain other feminists, however, because they represent the popular perspective on Nietzsche (if not that most typical of Nietzsche scholars) and because they indicate certain pitfalls that feminists would do best to avoid.Carol Diethe’s article “Nietzsche and the Woman Question,” for example, criticizes Nietzsche in a number of unwarranted ways. Consider, for example, her complaint that Nietzsche’s conception of the will to power entails an affirmation of the domination of women.If we take his key concept, the will to power, we find that it is so gender based that it practically comes to represent the archetypal phallic symbol. Bernstein describes it as a phenomenon which includes the “will to violate” so that the “feeling of power” (Machtgefühl) is generated, yet when he goes on to mention “Nietzsche’s desire to avoid reducing the will to power to sexuality,” he fails to realize that his own critique has relied heavily on the language of rape and the whole area of male dominance of women which stretches behind words such as “violation.” 1Diethe simply accepts Bernstein’s characterization of Nietzsche’s... Folgen wir dem Gedankenlauf, den der französische Philosoph Jean-Luc Nancy in seinen Werken The Sense of the World und Dis-Enclosure zum Vorschein bringt, dürfen wir die (...) Welt nicht als eine den Sinn innehabende auffassen, sondern müssen sie selbst als Sinn betrachten.
